We welcome both private and NHS patient referrals
Helping your patients to make us their hospital of choice is simple. If you are referring an NHS patient to us, simply use the NHS e-Referral Service (ERS). If you are referring a private patient to us, simply fill in the form below (please do not use this form if you are referring an NHS patient).
Your patients can select any one of our 10 Practice Plus Group hospitals or surgical centres nationwide, regardless of location.
