Terms and conditions 

Please read the following terms and conditions carefully. These terms and conditions govern the refer a friends & family scheme.  

1. Anyone who has had treatment in a Practice Plus Group Hospital or Surgical Centre [location list] is eligible to refer a friend or family member (“Referrer”). Initial consultation is defined as having an appointment with a consultant at a Practice Plus Group Hospital, which may also include a pre-operative assessment and an x-ray if required. The non-promotional price of this is £95.
2. To become a Referrer, you must use your unique referral code as provided to you on email. You can that pass this code to friends or family interested in receiving treatment at a Practice Plus Group Hospital or Surgical Centre. 
3. The referrer fee (£20 voucher) is only payable if the referred friend or family member physically attends an outpatient appointment, and quotes the referrers unique code upon booking. 
4. The discount will only be valid for the friend or family member referred to use on an initial outpatient appointment. It is not valid for use on enhanced diagnostic tests or procedures that take place during a consultation setting or radiology room.
5. As Referrer, you will not enter or otherwise use any information of third party or referred friend or family in order to use the Services for any bulk email distribution, distribution to strangers, or any other promotion of a personal link in a manner that would constitute or appear to constitute unsolicited commercial email or “spam”.
6. We will not discuss the details of your friend or family member’s appointments, treatment or the outcome of their treatment.
7. Friends must use the referral code that you share with them when booking an initial consultation at the Private Patient Contact Centre. Initial consultations must be booked via the Private Patient Contact Centre by contacting either private.patient@practiceplusgroup.com or 0330 053 6441. The ability to book an initial consultation is based on criteria which will be discussed with the referee before booking. No referral fee or discount will be provided if an initial consultation is not possible for any reason or if the friend or family member does not attend. Offer valid from the day the email code is received.
8. An initial consultation does not guarantee that treatment will be available or possible. Suitability for surgical treatment is assessed at the initial consultation.
9. The £45 offer is only eligible to use on an initial consultation and cannot be used in conjunction with any other offer.
10. There is no limit on how many patients you can refer to Practice Plus Group Hospitals or Surgical Centres.
11. You will receive the referral fee (£20 Amazon Voucher) within 45 days of the friend or family member attending their appointment. The voucher will be sent by post or email.

13. In the event of fraud, abuse, and/or an error affecting the proper operation of the promotion, Practice Plus Group reserves the right to end or suspend the promotion; and/or amend these terms and conditions without notice and at their sole discretion.
14. There is no cash alternative.
15. Practice Plus Group reserves the right to refuse the issue of any reward to any referred friend or family or Referrer at any time.
16. Practice Plus Group reserves the right to vary any and all elements of this offer at any time without notice.
17. The referred friend or family reward cannot be claimed by the same person making the referral.
