Practice Plus Group is pleased to offer our NHS patients the ability to manage their treatment with us via a new online Patient Engagement Platform. 

Securely access your patient record online

If you are an NHS patient receiving treatment at one of Practice Plus Group hospitals or surgical centres, you will be able to access your medical records via the Patients Know Best platform, providing a single space where you can view, update and share your information to help inform the care you receive.

Patients Know Best gives you secure access to your health record

Benefits for our patients include:   

  • A secure online space to view your hospital letters and appointment details. 
  • The ability to amend or cancel appointments online, if needed. 
  • Access to a library of health resources to aid in managing your health and prepare you for surgery and support you in recovery after your procedure. 
  • The ability to share your record with family, carers and health professionals. 

The platform can link to a range of devices and apps including Apple’s Health app, Fitbit’s app and devices, and more – please note, information on third party apps will not be actively reviewed by a clinician.

How to register

You can register for your online record in one of two ways:

  1. register using your NHS login (geographical restrictions may apply), or,
  2. register via PKB’s platform – to register using this method, you’ll need your Invite Code and Access Code; you can find these on your patient enrolment letter.

Accessing your record once registered

Once you have registered for your Patients Know Best record, you can access it online anytime, here at my.patientsknowbest.com or via your NHS login (geographical restrictions may apply).

Step 1 – get enrolled

To get started, use the QR codes in our waiting areas, or look out for your PKB patient enrolment letter.

Step 2 – choose how to register

Sign up using your NHS login (where available), or via PKB platform – for this method, you’ll need your Invite Code and Access Code.

Step 3 – complete registration

Follow the online instructions to create your account.

Step 4 – view your records

Login via the NHS app or PKB platform to access or amend appointments, view letters, discharge notes and access helpful resources.

Step 5 – share with who matters

Easily share your information with family, carers, or healthcare professionals, if you choose to.

Step 6 – stay informed

Get all the information you need to turn up on time, ready for your appointment. Link your data with health apps like Apple Health and Fitbit.

FAQs

Who are PKB?

Patients Know Best (PKB) are Practice Plus Group’s chosen software provider to bring you access to your patient engagement platform (PEP).

PKB are the UK’s biggest provider of this type of service and work with dozens of NHS trusts.

At first the PEP will be used to send you appointments and letters as well as access to a digital library of patient resources!

How much will using this service cost me?

As a valued patient of Practice Plus Group the PEP will be available to you completely free of charge to use for as long as you like once you have registered; even after your treatment with us has finished!

Who is being invited to join the patient engagement platform (PEP)?

Great news! If you’ve had an NHS appointment with us in the past 12 months, or are scheduled to have an appointment with us in the near future, you will receive an invitation to join the PEP automatically.

My treatment has finished – why have I been sent an invite?

If you had an NHS appointment with Practice Plus Group in the past 12 months we have invited you to register for the PEP.  This is so that you have a record of the treatment that you received from us.  It also means that you are already registered if you decide to see us as a patient again in the future.

What happens if my personalised invitation link has expired?

Personal invitation links only last for 10 days, so if this has expired not to worry! You can still create an account using the ‘how to register’ button on this screen or by clicking following link: Register for your record here.

How do I change my demographics/personal information?

To change your demographic profile, you will need to contact your hospital team to have them change it for you. We ask you to do this because verification is an important part of our process to ensure that all medical data including messages and lab tests are being sent to verified individuals. Having trouble logging in?

Have you tried the manual reset option? Click on the following link to do this: https://my.patientsknowbest.com/forgotPasswordForm.action. As a reminder our password criteria are below in case this helps to jog your memory:
 
10 or more characters long.
Contain at least one uppercase letter, one lowercase letter, one special character and one number.
Valid special characters: !”#$%&’*+,-.\:;<=>?@[]^_`{|}~
 
If your security information is not being accepted, we will need to reset this for you, you can request this by emailing: PatientPEP@practiceplusgroup.com.

Is my data secure?

Yes, your data is secure.  It is encrypted to the highest standards and only you can see your information. 

What do I do if I register but decide I no longer want my account?

You just need to let us know at the following email address and we will arrange this for you: PatientPEP@practiceplusgroup.com
